Appointment Process

New patients are required to complete the intake request form.

For in-network patients, we will provide an available appointment date along with all necessary forms required for the visit.

After submission, the patient will receive an email confirming whether their insurance is in-network. For patients whose insurance is not in-network, a self-pay option is also available.

Appointments not confirmed or forms not submitted at least 24 hours prior to the scheduled time may be automatically canceled or rescheduled.

Spruce is a secure telehealth app that makes it easy for providers and patients to communicate through messaging, video calls, and file sharing. It helps streamline care, keep conversations organized, and ensure HIPAA-compliant communication.

These are the billing platforms used by the practice. Patients are required to set up an account prior to their scheduled appointment to avoid billing issues.
